Completing your personal details

The personal details page is the first part of your application where we ask for information about you. Please use the name that you are known by at work as this is the name that will appear on the Register.

You must answer all of the required questions that have a small red star next to them for that section of the form to be complete.

Once you have completed all of the required questions on the page and click save and next at the end of the page, you should see a red tick next to the words personal details on the side menu showing you that you have successfully completed that section of your form.

If you need to go back to a section to change something you can do this any time before you submit your application by clicking on the section title on the side menu.

If on any of the options on the side menu you see a red triangle with an exclamation mark it means that you have not fully completed a part of your application and some of the information we need is still missing.

All the information you provide on your form must be correct to the best of your knowledge and belief. Giving us incomplete, false or misleading information may result in Social Care Wales refusing to register you or in removal from the Register.
